Don't speak "no focus" even when there is no accessible application focused

Users are complaining about spurious "no focus" statements. Due to the
relatively short timeout designed to inform the user when they have
landed in an application where Orca cannot work. However, increasing
the timeout to eliminate instances where the message is about to become
invalid means users likely won't get the notification before they alt+tab
or do something similar to get speech back.
