Change the Orca+b binding from "goToNextBookmark" to "flatReviewSayAll"
in NVDA and JAWS profiles if the user didn't make a modification that
would conflict (e.g. didn't change the binding for either and didn't
use that binding for something else).

When an app was emitting a window:deactivate after the app switcher
started talking, it would stop its speech because the app switcher
isn't considered as being the current app, and thus the logic to stop
speech of the current app on window:deactivate would kick in.

Work around this by removing the focus when the switcher gains focus,
which prevents the window:deactivate speech cancellation from kicking
in, without introducing too many side effects from actual window
switching.

This requires an extra patch not to speak "layered pane" on the
desktop.

The upstream patches don't seem to be enough with 3.24. Apparently
upstream is now using deprecated "focus:" event to work around some
toolkit issues with "object:state-changed:focused", and this code path
filters layout-only object early on, which the code path we have in
3.24 doesn't.  So, special-case presenting the desktop's layered pane.

Note: This requires the desktop to explicitly indicate it is the
desktop via the is-desktop object attribute.

Be explicit when a character should be spoken as a character.

This will be needed in the next patch.

This follows the XKB setting and its meaning, that separate press on
the modifier and a key will result in a combination of those.
